Visit http://fb.me/contact.form and click on Go To App. Fill up the details as per the instructions. You can add a header image to the contact form of width 519px. And make sure you keep the height less than 150px.
If you’re a CSS expert you can also customize the Contact form with custom CSS. You can also incorporate the contact form to your website by adding the iframe code. Just head over to the Export Tab.
